It is a state-licensed Adult Family Home designed to support those with developmental disabilities, brain injuries, or physical disabilities. Located on the Northside of Eau Claire allows it convenient access to community resources. The house is entirely wheelchair accessible and equipped with a roll-in shower. Caregivers are highly trained and are onsite 24 hours a day with awake supervision on overnights. |

Cadott: These supportive home care apartment settings are targeted to assist people with disabilities or advanced age that are just looking for a little extra support while maintaining the pride of living independently. Three of the apartments are fully wheelchair accessible, including a roll-in shower. One and two-bedroom units available, garbage, lawn/snow care, and onsite washer & dryer usage included. Assistance is available for medication management, transportation to medical appointments, grocery shopping, light cleaning, meal prep, and basic assistance with Activities in Daily Living. Staff is scheduled weekday hours based on individual needs. |

Holmen: This home is a state-licensed adult family home that supports up to four individuals. This home provides specialized support to people with Autism and/or Developmental Disabilities. The opening would be perfect for someone needing more space as it is located on the lower level of the home with a sensory room on the same level. There is an opportunity to attend day programming as well. Caregivers provide 24-hour awake supervision. |

Employees are highly trained to support individuals with complex medical needs, and a Registered Nurse Consultant is available to provide medical training and check-ins. |

Milwaukee: This home is a state-licensed Adult Family Home that can serve up to four men. The caregivers are specially trained to support individuals with Mental Health and/or Developmental Disabilities. Individuals in the home are active in the community so weekday staffing is not provided while they are not in the home. The house is in a quiet neighborhood conveniently located close to a major highway and shopping with a company vehicle available to provide transportation. The home features a large backyard with a patio, two living rooms/common areas, and a finished lower level. Caregivers are onsite when individuals are home with awake overnight supervision. |

The home is wheelchair accessible with roll-in showers, Hoyer lifts, and a wheelchair-accessible vehicle. A Registered Nurse Consultant is available to offer oversight and guidance when the need arises. |

The home is located in a quiet neighborhood close to the zoo, parks, and shopping. It features a large backyard with a deck that overlooks a large wooded area, a basketball hoop in the driveway, and large bedrooms. There is a vehicle on-site to provide transportation to outings and appointments. The employees are highly trained to support each individual’s needs and are onsite 24 hours a day with awake overnight supervision. |

This would be a perfect setting for a nature lover as it is nestled on a wooded lot with lots of wildlife. Although the area has a country feel to it, it is located close to the community and its amenities and resources. Caregivers are onsite 24 hours a day outside of day programming hours and they provide awake overnight supervision. |

The apartments are wheelchair accessible with roll-in showers. The Employees provide overnight awake supervision and are highly trained to support the individual’s needs. A Registered Nurse Consultant is also available to provide medical oversight and assist when unusual medical concerns arise.
